The Change Model reflects your personal experiences with implementing change combined
with the information discussed in class, your model presentation, the textbook, and article review. Do not use personal examples; rather this should be written in a format another change
practitioner could execute against. In addition to the outline, include a visual image of your model which you have designed. You should be able to hand this model to someone and they
could execute against it. Be creative!
Your change model is in OUTLINE format and provides specific data relevant to each of the
following areas from the text book:
1. Upstream Change – What are the critical areas that must be considered in order to
successfully plan second order change?
2. Midstream Change – What are the critical areas to pay attention to during the
implementation of change?
3. Downstream Change – What are the critical areas that assures change is achieved as
planned and is sustainable?
The paper should be in OUTLINE format, a minimum of 6 pages in length, double-spaced
and include a visual representation of your change model. No title page or reference page
